Applicants to the Miami Diaconal Program must complete the application for entrance to St Tikhon's Seminary.  These forms may be downladed at http://www.stots.edu/application_materials.html

Students must complete and submit all paperwork except for the medical forms (only required for students living on campus in Pennsylvania). On the application form, the program chosen should be "Other"; write in "Miami Diaconal Program" here. All paperwork should be returned to the following address for processing:
